I, the undersigned Pet Owner, am voluntarily allowing my pet to participate in services provided by Pawsandpalm, including transportation in the Pawsandpalm Dog Bus and related rides, walks, outings, excursions, recreational activities, and other pet-related activities.
I understand that participation in these activities involves inherent risks to my pet, including, but not limited to, transportation-related incidents, sudden stops or movements, interactions with other animals, unfamiliar environments, slips, falls, escape, injury, illness, allergic reactions, exposure to parasites or infectious diseases, heat or weather-related conditions, and other unforeseen circumstances.
I voluntarily accept these risks on behalf of myself and my pet.
I understand and acknowledge that my pet's participation in Pawsandpalm's Dog Bus services and excursions is voluntary and at my own and my pet's risk.
I understand that animals can behave unpredictably, even when they have no previous history of aggression, anxiety, escape, or other behavioral issues.
I accept responsibility for the risks associated with my pet participating in transportation, walks, excursions, and activities with Pawsandpalm.
I represent that, to the best of my knowledge:
I agree to notify Pawsandpalm promptly if my pet's health or behavior changes in a way that may affect participation.
If Pawsandpalm believes that my pet requires immediate veterinary attention, I authorize Pawsandpalm, its employees, contractors, handlers, or representatives to seek emergency veterinary care for my pet when reasonably necessary.
I understand that Pawsandpalm will make reasonable efforts to contact me or my emergency contact as soon as reasonably practical.
I authorize veterinary professionals to provide emergency examination, treatment, stabilization, or other care reasonably considered necessary for my pet.
I understand that I am responsible for all veterinary, medical, transportation, treatment, medication, and related expenses incurred on behalf of my pet.
To the fullest extent permitted by applicable law, I release and agree to hold harmless Pawsandpalm, including its owners, employees, contractors, drivers, handlers, agents, representatives, and service providers, from claims, demands, damages, costs, expenses, or liabilities arising out of or related to my pet's participation in Pawsandpalm's Dog Bus transportation, rides, walks, excursions, or related activities, including claims relating to injury, illness, escape, property damage, or other loss, except to the extent caused by conduct that cannot legally be waived or released under applicable law.
I understand that this waiver is intended to apply to risks inherent in or reasonably associated with pet transportation, animal handling, walks, excursions, and recreational activities.
I understand that my pet may encounter other animals during Pawsandpalm activities.
I acknowledge that even animals that have previously behaved appropriately may react unpredictably in a new environment.
I accept the inherent risks associated with interactions between animals and agree that Pawsandpalm is not responsible for injuries, illness, or other losses arising from such interactions except to the extent liability cannot legally be waived.
I understand that reasonable precautions will be taken to prevent my pet from escaping or becoming lost. However, animals may unexpectedly escape through doors, gates, equipment failure, or other circumstances.
I agree that Pawsandpalm will not be responsible for an escape, disappearance, or loss of my pet except to the extent caused by conduct for which Pawsandpalm cannot legally disclaim responsibility.
I agree to provide accurate and current contact information and any information that may assist in safely handling or identifying my pet.
I understand that any personal items provided with my pet, including collars, leashes, harnesses, clothing, toys, beds, blankets, food containers, or other belongings, are brought at my own risk.
Pawsandpalm is not responsible for loss or damage to personal belongings except to the extent required by applicable law.
I agree to provide Pawsandpalm with complete and accurate instructions regarding any medication, feeding schedule, allergies, special handling requirements, or other needs of my pet.
Pawsandpalm may decline to administer medication or perform a task if it reasonably determines that doing so would create an unsafe situation or requires professional veterinary administration.
Pawsandpalm reserves the right to refuse, suspend, modify, or end transportation or participation in an activity if, in its reasonable judgment, my pet's behavior, health, weather conditions, equipment, or circumstances create a safety concern for my pet, another animal, a person, or Pawsandpalm personnel.
I understand that such a decision may be made for safety reasons and does not necessarily indicate that my pet has done anything wrong.
I understand that walks, excursions, and outdoor activities may involve exposure to heat, humidity, rain, storms, cold, uneven terrain, insects, plants, water, and other environmental conditions.
Pawsandpalm may modify, shorten, postpone, or cancel an activity when conditions are determined to be unsafe.
I remain responsible for my pet at all times outside of the specific services being provided by Pawsandpalm. I agree to promptly inform Pawsandpalm of any changes to my pet's health, behavior, vaccination status, medications, emergency contacts, or other information relevant to safe participation. I also agree to follow Pawsandpalm's reasonable safety instructions and policies.
I understand that I am responsible for applicable Pawsandpalm service fees and for reasonable expenses incurred on behalf of my pet, including emergency veterinary care, transportation, medication, treatment, or other necessary services.
